Having a loved one admitted to hospital can be an anxious time. Our Hospital Liaison Officer is there to ensure carers feel welcomed and as relaxed as possible in hospital settings. She will work closely with hospital and social care staff to ensure loved ones can return to their own home as soon as possible. This is particularly important if someone is on an end-of-life pathway and has advised that they would like to die in their own home.
Our Hospital Liaison Officer works across a number of hospitals in Gloucestershire and;
- Provides or signposts carers to information, advice and support.
- Raises the profile of carers with hospital staff.
- Ensures the carer is included in conversations about their cared-for.
